چکیده

A 52-year-old man presented to the outpatient clinic with dyspnea on exertion and atypical angina. Cardiovascular risk factors were a history of smoking (25 pack years) and hypertension, and the pretest risk probability for coronary artery disease was 49%. The coronary artery calcium score (CACS) was 0; however, because of symptoms coronary CT angiography (CTA) was performed which showed a severe non-calcified lesion in the mid left anterior descending artery (LAD), Figure 1. Sequentially, an adenosine stress CT myocardial perfusion (CTP) was performed showing a reversible, anterolateral ([50% transmural) perfusion defect, Figure 2B. Invasive coronary angiography confirmed the significant stenosis in the mid-LAD (Figure 3), which was stented. Due to persisting symptoms single-photon emission computed tomography (SPECT) myocardial perfusion imaging (MPI) was performed, which showed dissolved ischemia anterolateral without new perfusion abnormalities, Figure 4.

Coronary artery calcium (CAC) score – a prognostic tool in coronary artery disease?

Introduction: The aim of this study was to evaluate the impact of measurement of coronary artery calcification score (CAC) in patients with suspected coronary artery disease (CAD) and a normal myocardial perfusion scan. Methods: In a prospective study we measured the calcium score of 74 patients (29 m, 45 f, mean age 58.7 (m) and 64.4 (f)) with suspicion of CAD and a normal perfusion scan. In ...

One-stop-shop cardiac CT: 3D fusion of CT coronary anatomy and myocardial perfusion for guiding revascularization in complex multivessel disease

A 59-year old male, with a history of inferoposterior myocardial infarction and multiple coronary stenting, presented to the out-patient clinic with exercise-related chest discomfort. The electrocardiogram showed sinus rhythm with Q-waves in the inferior leads (Figure 1). Coronary computed tomography angiography (CTA) showed stents in the right coronary artery (RCA), left anterior descending ar...

A Comprehensive Computed Tomography Protocol for Evaluating Left Ventricular Systolic Dysfunction Un protocolo integral de tomografı́a computarizada para evaluar la disfunción sistólica ventricular izquierda

The article by Estornell-Erill et al. published in Revista Española de Cardiologı́a investigates the ability of multidetector computed tomography (CT) to detect the causes of left ventricular systolic dysfunction (LVSD) in a single comprehensive assessment. In this study, the authors combine coronary artery calcium (CAC) scoring, anatomic luminal imaging of the coronary arteries by CT angiograph...

The prognostic value of stress/rest gated myocardial perfusion SPECT in patients with known or suspected coronary artery disease

Introduction:Gated myocardial perfusion SPECT has diagnostic and prognostic values in coronary artery disease (CAD). We tried to determine prognostic values of the left ventricular perfusion & functional indices as well as transient left ventricular dilation (TID) derived from gated myocardial perfusion SPECT. Methods: 1820patients who underwent gated myo...

Noncalcified atherosclerotic plaque burden at coronary CT angiography: a better predictor of ischemia at stress myocardial perfusion imaging than calcium score and stenosis severity.

OBJECTIVE The purpose of this study was to examine the relation between the coronary CT angiographic findings of calcified and noncalcified plaque burden and stenosis severity and the myocardial perfusion imaging finding of ischemia. MATERIALS AND METHODS Seventy-two patients (41 men, 31 women; mean age, 56 years) underwent coronary CT angiography and stress-rest SPECT myocardial perfusion im...
